When are inactive airlines removed from a world, and do they keep your fares down? (public or private)

3 weeks in total. After 2 weeks they get a warning and then they have 1 week to reset the timer before they are reset in the world.

Yes and no. You will need to manually increase the fares after competition is removed. But yes, you can increase the fares if the competitor(s) pull out.

Correct. They spent tokens joining the world so they won't be forcibly removed.
